Hospice General Criteria Guide
At Dignity Hospice, we believe every family deserves care that honors comfort, dignity, and peace. If you notice two or more of the following signs in your loved one, it may be time to begin a conversation about hospice care. Please consult with your physician to discuss the most appropriate plan of care.
Signs That Hospice May Be Appropriate
- Increased need for assistance with Activities of Daily Living (bathing, dressing, eating, mobility)

Unintentional weight loss despite adequate nutrition

Changes in mental status such as confusion, memory decline, or reduced awareness

Multiple falls or increasing frailty

Skin breakdown or slow-healing wounds

Frequent medication adjustments to manage symptoms

Shortness of breath that is worsening or limiting daily activity

Repeated emergency department visits or hospitalizations

Recurrence of multiple infections

Symptoms that are becoming harder to manage

Treatments that are no longer effective and/or the patient expresses a desire to stop aggressive interventions

Presence of other diagnoses contributing to overall decline
